Conducted a value chain and gap analysis and produced a set of recommendations for how value based care organizations can partner with Geriatric Emergency Departments. The work was highlighted in a Health Affairs article, and reviewed on a webinar and podcasts (part 1 & part 2).

Created a 42-page operational Toolkit for value based care organizations to forge aligned partnerships with their local Geriatric Emergency Departments. The Toolkit contained a four-step framework and 22 corresponding tools and templates.

Provided project management support for Piloting the Toolkit with four value-based care organizations and geriatric emergency departments. The outcomes of the Pilot were highlighted in an Institute for Advancing Health Value case study and webinar.
